Purpose of an NDIS Plan Review Report
An NDIS plan review report is a structured piece of clinical evidence written specifically to inform an upcoming NDIS plan reassessment, an unscheduled review, a plan variation request or a change-of-circumstances submission. Unlike a Functional Capacity Assessment — which establishes the participant's baseline functional profile — a plan review report focuses on what has *changed* since the last plan, what is currently working, what is not, and what needs to change in the next plan.
A high-quality plan review report is the single most influential piece of evidence in a participant's reassessment meeting. It gives the NDIA delegate clinical justification for adjustment, helps Support Coordinators advocate effectively, and reduces the risk of a plan being rolled over unchanged when the participant's circumstances have evolved.
