Our Quality Assurance Process.

Every report we deliver passes through five quality gates before it reaches the participant or Support Coordinator. Here is what each one does.

Gate 1 — AHPRA registration verification.

Every assessing clinician is verified against the AHPRA national register before being engaged on any matter. We re-check registration annually and document the check date in our internal QA log. No clinician with a registration condition, undertaking or restriction is engaged on an FCA without that fact being disclosed in writing to the participant and referrer beforehand.

Gate 2 — Standardised tool integrity.

Every clinician on our roster is trained in the standardised tools they administer. Tool scoring follows the published manual without modification. Where a tool has a paediatric and adult version, we use the version that matches the participant's age. Where a tool has multiple translations, we use the AU-English version unless a culturally-validated alternative is more appropriate for the participant. Tool scores are reported with their published interpretation, not adjusted to fit the recommendation.

Gate 3 — Peer review on every report.

A second AHPRA-registered clinician reads every draft report before delivery. The peer reviewer checks specifically for: (a) recommendations that don't follow from the documented evidence, (b) inconsistencies between sections, (c) missing NDIS Act mapping, (d) over-claiming or under-claiming relative to the assessment data, and (e) clarity for the non-clinical reader. Peer-review feedback is acted on before the report is delivered.

Gate 4 — Audit-ready record-keeping.

All clinical records — intake forms, video session recordings (where consented), standardised tool scoring sheets, collateral correspondence and draft + final reports — are stored encrypted at rest in Australian-hosted infrastructure compliant with the Privacy Act 1988 and the Australian Privacy Principles. Records are retained for a minimum of seven years from the date of report delivery. Any record can be produced on subpoena, on AAT request, or on the participant's request within 14 days.

Gate 5 — Continuous clinical development.

Every clinician on our roster maintains their AHPRA-required CPD hours plus additional internal training in NDIS evidence standards, the NDIS Act reasonable-and-necessary framework, and updates to standardised tool guidance. Internal case conferences run quarterly where complex cases are reviewed by the full team to maintain consistency of clinical reasoning across the practice.

What happens if you disagree with a finding.

If a participant, Support Coordinator or referrer disagrees with anything in a final report, we offer a free review. We re-read the report against the evidence pack, document our reasoning, and where the disagreement points to a genuine error or omission, we issue a corrected version at no charge. Where the disagreement is about clinical interpretation, we explain our reasoning in writing and the participant retains the right to seek a second opinion.
