Why Neurological Conditions Need Specialised Assessment
Participants with neurological conditions present uniquely complex functional profiles. Motor, cognitive, behavioural, fatigue and self-management impairments interact in ways that are not captured by domain-by-domain assessment alone. A specialised neurological FCA integrates all of these dimensions into a single coherent functional narrative.
Acquired Brain Injury (ABI)
ABI participants present with combinations of motor, cognitive and behavioural impairment that depend on the location and severity of the original injury. Cognitive impairment — particularly executive function — often outweighs physical impairment as a driver of functional need. Behavioural change (disinhibition, impulsivity, emotional dysregulation, lack of insight) is common and substantially affects community participation and safety.
Multiple Sclerosis (MS)
MS is characterised by significant variability across the week and a progressive trajectory across years. Many MS participants have fatigue as the dominant functional limiter — not motor weakness or sensory change. A defensible MS FCA explicitly captures fatigue patterns, the impact of heat and stress on functioning, and the typical progression trajectory over the next 12 months.
Parkinson's Disease
Parkinson's introduces ON/OFF medication cycles, motor fluctuations, freezing episodes, and progressive cognitive change. A defensible Parkinson's FCA captures functioning across both ON and OFF periods, and addresses the gradual loss of insight and executive function that often accompanies the motor symptoms.
Stroke
Stroke produces extraordinary variability depending on the affected region. Right-sided strokes may produce neglect, spatial impairment and impulsivity; left-sided strokes may produce aphasia and apraxia. A defensible stroke FCA captures these specific patterns rather than treating stroke as a single homogeneous condition.
Motor Neurone Disease (MND)
MND has the most rapid functional trajectory of any common neurological condition — participants frequently move from independent to high-support within 12–18 months. Early assessment is critical because funding cycles cannot keep up with deterioration speed. Our MND assessments document projected 6-month and 12-month trajectories alongside current functioning.
Why Physiotherapy-Led Assessment Adds Value
Physiotherapy-led neurological FCA brings specific depth in mobility analysis, transfer mechanics, falls and risk assessment, fatigue profiling, pain-function interaction, and assistive technology justification. For participants whose functional picture is heavily shaped by physical capacity, falls risk or fatigue, the physiotherapy lens produces more accurate quantification than non-physiotherapy-trained assessment.
The Care and Needs Scale (CANS)
The CANS is an 8-level supervision scale originally developed for ABI and now widely used across high-support neurological cohorts. CANS 5–6 typically maps to 1:1 daytime support; CANS 7–8 typically maps to 2:1 or active overnight. The CANS is a powerful supplementary tool in neurological FCA, but should always be used alongside — not instead of — domain-by-domain functional assessment.
